Creek Chub Injured Minnow with 5 Hooks?

Danny E Hill	While going through my father's collection, I came across a 5 hook lure that appears to be a Creek Chub Injured Minnow but it has 5 hooks instead of 3. If it is actually a Luckey's 5th edition it very rare and was never cataloged. I'm attaching photos of it and asking help to see if I have identified it correctly. Thanks, Dan Hill

Bruce Dyer	It appears to have 2 weights. If so, rather than being an Injured Minnow, it would be the #1800 I'd think., Life Member 101.
